Player Forms Due at First Practice
At your first practice please collect 2 copies of the player form and verify each player's age on their birth certificate.  Remind each parent to bring these.  Do not collect the birth certificate.  Keep 1 copy of the player form with you at all times and forward the other on to your division coordinator.
Volunteer Forms
Every coach (head coach and assistant coach) must provide the following form every year:
  2. Select Login or First Time (this is a different system from the player registration)
  3. Select (My eAYSO, top left)
  4. Register yourself as a volunteer for this year (if you did it last year, you must re-register again)
  5. Print the volunteer form
  6. Show your Driver’s License to any AYSO Board Member (this included Division Coordinators) and have them sign this volunteer form. Board members will be available at each major event, they are also listed online: http://ayso1282.d4sportsclub.com/PageCustom.aspx?id=78
